These forms are a basic PDF file, which can be viewed on screen using Adobe Acrobat Reader and printed. You will need to print the form and complete it by hand.
You can also save the file to your hard drive (choose File/Save As... after it has opened) and view it through Adobe Acrobat. Please check back here periodically though to make sure you still have the most up to date version!
